Driving Population Health Management with Visual Analytics at Mount Sinai Health

For most healthcare organizations, the concept of working together to improve care at the lowest cost is ideal, but the new standards of operation under the Affordable Care Act (ACA) are understandably daunting to execute. In this relatively new territory, many organizations still struggle to understand just how to improve their care gaps, assess risk, connect with patients post-discharge and deploy population health monitoring to treat chronic diseases within regions and communities.

How can your organization embrace this new value-based care model and integrate population health management into process, work culture, and business systems?

See how Mount Sinai Health System, one of the largest healthcare systems in the U.S., empowered physicians and knowledge workers alike to ask and answer their own questions with data, visually.
